What the inspector wrote

CASE MANAGEMENT INSPECTION CONDUCTED On February 24, 2026, at 9:20 AM, Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Case Management inspection to follow up on an incident that was reported to the Department on 2/3/26. Upon arrival, LPA met with facility representative Julia Toscano, who provided LPA on a tour of the facility. Census was taken. On 2/3/26, an incident was reported to the department where a child fell and hit the table. The facility reported this incident to the Department within the required time frame. During this inspection visit, LPA interviewed staff who observed incident. S taff accounts of the incident revealed the child was playing alone, got up to walk to the table and tripped over the foot of another child. Staff immediately tended to the child's wound and notified parents. LPA observed the indoor classroom where the incident took place but furniture has been rearranged since the incident. LPA obtained and reviewed documentation. Based on the information obtained and LPA observation of the play area, LPA determined there was adequate supervision provided the day of the incident. The facility was found in compliance per Title 22 regulations, no deficiencies will be cited today 2/24/26. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and report was reviewed with facility representative, Helia Castellon.
