What the inspector wrote

On 05/22/26, at 11:15 AM Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection due to an incident that occurred on Monday, 05/18/26. This facility is one of Glendale Unified School District sites. The program operates AM and PM in Room 1105. AM program operates Monday- Friday 8:30 AM to 11:30 AM, and The PM program operates Monday - Friday 12:30 PM to 3:30 PM. LPA arrived at the facility, entered through school main entrance and was guided to the preschool 1105 classroom. Upon arrival in Room 1105, LPA met with teacher Lucia Derbedrossian and waited in the classroom 1105, shortly after, Alyson Beecher, Program Supervisor arrived to the classroom who guided analyst on a tour of the facility. Upon arrival, there were 15 children present in the classroom with 3 staff for AM session. During this inspection at 12:30 PM, 6 children arrived in the classroom for PM session with 2 staff. Alleged Incident took place on Monday 05/18/26, child#1 disclosed it to staff#3 on Tuesday 05/19/26. Program Supervisor was informed about the incident next day on Wednesday 05/20/26 and reported the incident same day on Wednesday 05/20/26 via email. Original LIC 624 Unusual Incident/Injury Report form was received by the Department within 7 days. The facility reported the incident within the required 24 hour time frame from the time that staff#3 learned about the incident. LPA conducted interviews with four staff, one child and one parent. LPA took pictures from classroom. At this time, the above incident require additional information to conclude this investigation. LPA will conduct a follow up inspection. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Alyson Beecher, Program Supervisor.
