What the inspector wrote

Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management licensee Initiated inspection at the above facility on 04/30/26 at 01:45 p.m. At 01:50 p.m., LPA met with the licensee Pingna Guan who guided LPA on a tour of the facility and census was taken. Also present was the licensee’s husband/assistant. The purpose of the inspection is to add the front yard as an outdoor play area that was previously off-limits. According to the licensee, construction of an ADU near the backyard will begin sometime next month and decided to place the backyard as an off-limits area. Prior to construction, children were utilizing the backyard for outdoor play. Licensee provided a written safety plan via email. Licensee will ensure that children do not have access to the backyard during construction, will inspect the front yard prior to children use to make sure there is no hazards or debris, children will be supervised at all times, and will ensure that children do not have contact with construction workers. Licensee and assistant guided analyst to the front yard where LPA observed that the front yard is fully fenced and has two ways to access the front yard and the entrance of the home. One entrance is through the driveway and the other entrance through the front near the sidewalk. According to the licensee, parents and children will enter through the entrance near the sidewalk. There is natural shade provided by trees and licensee stated that she purchased two umbrellas that will be delivered today to provide more shade. Licensee also stated that the children’s toys, materials, and equipment will be moved from the backyard to the front yard. LPA did not observe any hazards in the front yard. Licensee will submit pictures to LPA once toys and equipment for children have been added to the front yard. Licensee understands that the department shall be notified once construction has been completed, if use of the backyard for outdoor play will resume, and an updated facility sketch LIC 999. Page 1 of 2 At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today. The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative, a civil penalty of $100 can be assessed. An exit interview was conducted, and a copy of this report was provided to the licensee Pingna Guan. Page 2 of 2
