101238 Buildings and Grounds(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.This requirement is not met as evidenced by: Based on observations in room K1: the ABC rug is lifting/curling, and the cement ramp with railings made of wood was removed; however, it will be replaced until Friday April 17 after operation hours.Room K2: the wood in the entrance of the room feels deep and soft when walking in/out. The edge of the floor has a gap and exposes the wood which is dark brown not the same color as the rest of the wood in the classroom. Light bulbs are either missing or not working. The light bulbs are also missing their covers which are currently exposed.
