What the inspector wrote

At 09:20 a.m. Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced POC (Plan of correction) inspection to ensure the Type B deficiencies cited on 04/15/26 have been cleared. At 09:30 a.m. LPA met with Marisa Armenta, facility representative and census was taken. The following was observed: -Staff 2 (S2) provided proof of MMR vaccine. -The ABC rug is still being used and the facility representative disclosed that rug tape was continued to be placed under the rug; however, it only lasted for 3 days which still caused the rug to lift/curl all around. During the inspection, the facility representative removed the ABC rug a nd stated that the rug will not be used. LPA discussed with the facility representative that the rug is not safe for children as it poses a tripping hazard and could cause an injury. LPA also recommends that the facility has a back up rug to replace once the one in use is not in a good condition. -The cement ramp with railings was replaced and was observed to be properly installed. -According to the facility representative, it was determined that room K2 will be removed from the license since the preschool program is no longer using the classroom, and capacity decrease application will be submitted to the Monterey Park Regional Office on 06/11/2026. Currently, the facility has completed the LIC 200A, LIC 610, and a written letter with the facility letter head and is pending LIC 999 and a capacity increase/decrease fee. Page 1 of 2 The following deficiencies were cited in accordance with Title 22 of the California Code of Regulations and Health & Safety Codes. Please see 809D for documentation of deficiencies. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
