What the inspector wrote

Licensing Program Analysts (LPAs) Jeanette Estrada and Jonnisha Culbert made an unannounced Case Management Incident visit at the facility to follow up on a self-reported incident that occurred on 4/15/24 in regards to a child's personal rights concern. The Monterey Park SouthWest Office received the Unusual Incident Report via phone on 4/17/24. LPAs met with Site Supervisor Maria Olmeda and informed her of the reason for the visit. Upon arrival there were no children present. The P.M. session began at 12:00PM and there were a total of 13 children and 3 staff present in the classroom. On 4/17/24 it was reported that on 4/16/24 the facility reviewed a message from the parent of child 1 that had been sent on the Learning Genie App on 4/15/24 at 10:41PM. The message included photos of Child 1's arm with what appears as a nickel sized bruise. Per the parent's message, Child 1 stated that Staff 1 was not nice and answered "teacher" when asked who hurt him by the parent. The facility conducted an investigation and interviewed all staff present on 4/15/24. No incidents were reported by any of the staff. During today's visit, LPAs conducted staff interviews and collected pertinent documents. Further investigation will be conducted by CCLD. No deficiencies cited at this time. Notice of site visit was provided and Site Supervisor was reminded it must remain posted for 30 days. Exit interview conducted and a copy of the report was provided to Site Supervisor, Maria Olmeda.
