What the inspector wrote

Licensing Program Analysts (LPAs) Jonnisha Culbert and Jeanette Estrada made an unannounced Case Management Incident visit at the facility. The purpose of this inspection is to follow up on an unusual incident that was reported to the Department on 4/10/2024. There were 17 children and three staff present during today's visit. LPAs met with Site Supervisor Maria Olmeda and informed her of the reason for the visit. LPAs conducted a follow up visit regarding the incidents previously on 04/16/2024. During the visit, LPAs conducted interviews with Staff and spoke with a Detective on 5/13/2024. The incident reported occurred on 04/16/2024. The incident reported involved Child 1 and Staff 1. During the visit on 04/24/2024, Staff interviews were conducted. Per staff interviews, no one witnessed Staff 1 mishandling Child 1 at any time. Staff 1 denies mishandling Child 1. After interviewing Staff, Site Supervisor expressed that the facility was visited by local Law Enforcement regarding incident. LPA contacted the Los Angeles County Sheriffs Department (LACSD) and spoke with Detective Lizarraga who informed LPA that no evidence was found that supported the allegation and that the case was going to be closed due to parents not wanting to move forward in criminal investigation. Based on Staff interview and record reviews no deficiencies were issued during this visit. Notice of site visit was provided and Site Supervisor Maria Olmeda was reminded it must remain posted for 30 days. Exit interview conducted and a copy of the report was provided to Site Supervisor Maria Olmeda.
