What the inspector wrote

Licensing Program Analyst (LPA) conducted an unannounced case management inspection at the facility. LPA met with Site Supervisor Gloria Torres and advised the reason for the visit was to follow up on the Unusual incident Report (UIR) that occurred on 7/7/25, and submitted to the Department on 7/8/25. There were 21 students and 5 staff in room 2. During this visit, LPA conducted staff interviews and collected pertinent information. The incident reported that Child 1 who was left on the playground unsupervised. Per interviews conducted with Staff #1 - Staff #5 and Site Supervisor. At approximately 12:05pm on 7/7/25 Staff #1 discovered Child #1(C1) was not present inside the classroom upon taking roll for lunch. Staff #1 and #4, went to the playground and found C1 sitting on a blue sofa holding a book. Per staff, the child was observed to be unharmed and calm. Interviews conducted confirm the staff transitioned from the playground at around 12:00 noon. LPA observed that the playground is located approximately 20 feet from the entrance to the classroom, the sofa where C1 was reported to be found is around the corned and not visible to staff. Based on Interviews conducted and documents reviewed, C1 was left without direct supervision of staff for approximately 5-6 minutes. This poses an immediate risk to the health and safety of children in care. Based on interviews and record review the above incident is being cited according to CCR Title 22. A type A citation is being issued for regulation 101229 Responsibility for Providing Care and Supervision. _____ Continued on page 2 _____ LPA informed Facility Representative that this report dated 7/9/25 documents one (1) type A citation which will be posted for 30 consecutive days as there are immediate risks to the safety of children in care. LPA also informed Facility Representative to provide a copy of this licensing report dated 7/9/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the facility representative Gloria Torres, Appeal rights were discussed and provided
