What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry made an unannounced Case Management Incident visit at the facility to follow up on a reported incident that occurred on 10/10/25. The report indicated that Child 1 was injured inside classroom. The incident was reported as required within 24 hours. LPA met with Site Supervisor Gloria Torres and informed her of the reason for the visit. Upon arrival there were 14 children present with 3 staff. During this visit LPA conducted interviews with children and staff. Staff interviews stated that children were returning to the classroom in groups of 8, S1 stood at the door while the 1st group entered, C1 was second to enter the classroom and tripped and fell on chair causing cut. S1 stated they did not see the incident as they briefly turned their head to the students entering door. Child 1 immediately went to S1 when incident happened. Interviews with children confirmed that C1 tripped over child's own shoe and fell on chair. Records reviewed show that on date of incident staff and child ratios were in compliance, C1 was provided with 1st aid and parent was notified within 5 minutes of incident. LPA observed that there are no tripping hazzard in the location where child fell. Per site supervisor, they have taken precautionary measures by having an additional staff inside the classroom when children first enter after recess. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no deficiencies cited issued today. Notice of site visit was provided and Site Supervisor was reminded it must remain posted for 30 days. Exit interview conducted and a copy of the report was provided to Site Supervisor, Gloria Torres.
