What the inspector wrote

On July 9, 2025, at 12:21p.m., Licensing Program Analyst (LPA) Keneisha Dunlap conducted an unannounced case management visit at the above facility. LPA Dunlap announced the purpose of visit and was granted entry into the facility by the Licensee- Yolanda Stowe. The purpose of today's visit was to follow up for the facility’s required visits. The Licensee was on site at the time of the inspection. Physical Plant: Indoor Space LPA Dunlap observed the Classroom where School Age Children are located. The classroom was equipped with age-appropriate furniture and learning materials. Classroom has carpeted area, soft furnishings, age-appropriate toys, dramatic play, manipulative items, and a library area. At 12:35p.m., LPA Dunlap observed the School Age Classroom does not have a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity. The Licensee was informed that they must provide the classroom with adequate materials, as this poses a potential health and safety risk. A Technical Assistant will be cited. At 12:45p.m., LPA Dunlap observed a broken table in School Age Classroom. The table had a book wedged underneath the leg of the table to prop up. The Licensee was informed that the broken table must be immediately removed from the classroom and either replaced or repaired, as this poses a potential health and safety risk. A Technical Violation will be cited. At 12:55p.m., LPA Dunlap observed a broken handle on the faucet in School Age Bathroom. The Licensee was informed that the broken handle must immediately be replaced or repaired, as this poses a potential health and safety risk. A Technical Violation will be cited. Page 1 of 2 Physical Plant: Outdoor Space The Playground area has canopy for shade, small climbing structure with wood chip area, balls, basketball hoop, tether ball poll, and age-appropriate tables and chairs. At 1:05p.m., LPA Dunlap observed the School Age playground to have debris in the wood chip area. The Licensee was informed that the playground must be cleaned and free of debris, as this poses a potential health and safety risk. A Technical Violation will be cited. At 1:15p.m., LPA Dunlap observed the School Age Playground does not have a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity. The Licensee was informed that they must provide the playground area with adequate materials, as this poses a potential health and safety risk. A Technical Assistant will be cited. School Age area was in their designated classroom at the time of the visit. Staff #1 File has all required licensing documents in file CPR expiration date of 8/12/25 Mandated Reporter expiration date of 2/26/26 Accredited courses in file. A notice of site visit was given and must remain posted for 30 days. 3 Technical Violations issued during today’s inspection. 1 Technical Violation issued during today’s inspection.
