What the inspector wrote

On October 16, 2025, at 12:15 p.m., Licensing Program Analyst (LPA) Peter Bishop arrived at the above facility to conduct an unannounced case management visit. This is a School Age Program. There were 0 School Age children present at the time of this visit. LPA Peter Bishop announced the purpose of visit and was granted entry into the facility by the Director - Yolanda Stowe. The purpose of today's visit was to follow up for the facility’s required visits. The Licensee was on site at the time of the inspection. Physical Plant: Indoor Space LPA Bishop observed the Classroom where School Age Children are located. The classroom was equipped with age-appropriate furniture and learning materials. Classroom has carpeted area, soft furnishings, age-appropriate toys, dramatic play, manipulative items, and a library area. LPA Bishop observed the School Age Classroom has a variety of age-appropriate equipment, toys and materials were observed to be in good condition and in sufficient quantity. At the time of this visit LPA Bishop observed the facility to be in compliance with age-appropriate equipment. LPA Bishop did not observe in broken or damaged furniture. Licensee indicated that all broken furniture had been removed and LPA did not not observe any broken or damaged furniture at the time of this visit. LPA Bishop found the furniture was in compliance. LPA Bishop inspected the bathroom for any potential hazards. LPA Bishop did not observe any deficiencies. LPA Bishop observed that there handles on the faucet in the School Age Bathroom. LPA Bishop found the bathroom to be in compliance. Page 1 of 2 Physical Plant: Outdoor Space The Playground area has canopy for shade, small climbing structure with wood chip area, balls, basketball hoop, tether ball poll, and age-appropriate tables and chairs. LPA Bishop observed the School Age playground. The playground was free of debris and LPA Bishop did not observe any potential health and safety risks at the time of this visit. LPA Bishop observed the School Age Playground to have a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity. The playground was found to be in compliance at the time of this visit. Staff #1 File has all required licensing documents in file CPR expiration date of 8/12/25 A Type B Deficiency will be issued today. Mandated Reporter expiration date of 2/26/26 Accredited courses in file. Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. 1 Type B Deficiency will be issued today. A notice of site visit was given and must remain posted for 30 days.     PERSONNEL REQUIREMENTS

    At Least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid persuant to health andsafety code 1596.866 shall be present when children are at the child care center or offsite for center activites. Based on record review 3 of 4 staff did not have a valid Pediatric CPR and First Aid approved by Emergecny Medical Services Authority EMSA which poses a potential health, safety or personal rights risk to persons in care.
