On 05/07/26 at 09:30AM, Licensing Program Analyst (LPA) J.Duran conducted an unannounced case management incident visit at the above facility. LPA announced the purposed of today's visit and met with Site Supervisor, Adriana Huerta. LPA toured the facility. There were a total of 14 children present during the inspection. Site Supervisor submitted a UIR to the Department on 04/23/26 regarding an incident that occurred on 04/23/26 stating that on 04/22/26 at approximately 10:48am, child was running towards a bike and fell and hit his knees and forehead on the ground. Daniel's left knee and forehead was scraped and he cried. Teacher Carolina, substitute Teacher Marilyn, and six students were present at the time of the incident. Teacher Carolina contacted Adriana for assistance. Teacher Carolina provided two ice packs one for his knee and one for his forehead and band aids. At approximately 10:52am mom was contacted. At 1pm, mom picked up Daniel. During today's visit LPA conducted interview with Site Supervisor and parent and conducted child and staff file review. LPA obtained facility roster. Based on today's visit, LPAs have no concern with the level of care and supervision. At this time, no further investigation will be conducted by CCLD. No deficiencies cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the Site Supervisor, Adriana Huerta.
