What the inspector wrote

Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced case management- incident inspection to the above facility. The purpose of this inspection was to follow-up on a incident that was self reported to the department on 5/27/2025. LPA met with Director Maricruz Flores- census was taken. On 5/27/2025 an incident was self reported to the department. Per Incident reported Child # 1 had an allergic reaction after having a piece of chocolate that was offered to them by another child. LPA conducted interviews with staff Regarding incident- Staff corroborated that they remind children to not share food with others, due to potential allergies. Per interviews child # 2 offered child # 1 a Dubai Chocolate that contained pistachios. As soon as child # 1 ate the chocolate they had a reaction. Per Staff, Child # 1 had no known allergies on file and parents were not aware of any allergies. Child # 1 was taken to urgent care- diagnosed with allergic reaction. Child # 1 now has an IMS plan and medication on cite. LPA advised Director to read the PIN 22-02- CCP IMS Best Practices. Facility is a nut free zone- they conducted a meeting with staff and parents, regarding sharing food and bringing candy to school At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Maricruz Flores. ---------------------- pg. 1 of 1 -----------------------
