What the inspector wrote

On 02/17/2026 at 8:30AM Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced case management- incident inspection to the above facility. The purpose of this inspection was to follow-up on a incident that was self reported to the department on 01/14/2026. LPA met with Director Maricruz Flores- census was unable to be taken as no school age children were present at the time of inspection. On 01/14/2026 an unusual incident report (UIR) was self reported to the department. Incident took place on 01/13/2026 and UIR was reported in a timely manner. The self-reported incident is regarding supervision and personal rights. Brief summary of incident: Child #1 (C1) was watching inappropriate content on an electronic device, and Child #2 (C2) observed the inappropriate content while walking around the school-age classroom, while under the supervision of Staff #3 (S3) and Staff #4 (S4). Page 1 of 2 During interviews, Parent #2 (P2) disclosed that the children were allowed to use their electronic devices throughout the span of time Child #1 (C1) was present at the facility. During this span of time, inappropriate content was viewed on Child #2 (C2's) device while under the care and supervision of Staff #3 (S3) and Staff #4 (S4). During interviews, P2 disclosed that as a result of viewing the inappropriate video on C1's device, C2 experienced an interference with functions of daily living. At this time, the facility is not in compliance with California Code of Regulations Title 22 and two type A deficiencies are being cited during today's inspection (See attached LIC809D). A notice of site visit was given and must remain posted for 30 days. Appeal rights were given and an exit interview conducted. The report was reviewed with Director Maricruz Flores. Page 2 of 2

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, Supervision shall include visual supervision.This requirement is not met as evidence by:Based on interviews conducted, C1 and C2 were left unsupervised by staff watching inappropriate videos on an eletronic device. This posed an immediate risk to the children in care.
