What the inspector wrote

This complaint is also addressing an incident reported to the Department on 06/09/2025. Staff interviews were conducted. Staff #1 was able to provide attendance information regarding Child #1, the week the incident may have taken place. Child sustained an injury from falling at the facility Monday 06/02/2025 and parent was contacted to be picked up early (incident report obtained). Tuesday, child returned to facility and when staff asked how child was doing, parent stated that they were "fine and just being dramatic." Wednesday, child was dropped off by relative but for reasons unrelated to report, was sent home early. Child was absent Thursday and Returned Friday. The following Monday, 06/09/2025, facility was informed of the child's allegations and child had bruises. During interview with Staff, it was disclosed that the Staff #4, involved in the report, works at multiple locations and their schedule was different each day. Staff #4 confirmed their work location during the week prior to the incident. Interviews with staff had no disclosures regarding Staff #4 and Child #1. Information from Staff indicates that nap time was the time of incident. Per Staff #3, Staff #4 is not near Child #1 during nap time and is not alone with children. Site Supervisor stated that the Sheriff Department visited. LPA attempted to reach out to the Sheriff but was unable to contact. Children interviews made no disclosures regarding allegation. Parent interviews made no disclosures regarding allegation. LPA attempted to contact child and parent involved in the report but was unable to conduct interviews. Based on the above information, there is conflicting information regarding the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. No citations are being issued during today's inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Site Supervisor Wendy Zhang, at 12:30pm. Copy of Report provided. END OF REPORT PAGE 2 of 2
