What the inspector wrote

On 6/20/25 a unannounced Case Management-Other visit. LPA met with Facility Representative Marcela Occhione and explained the reason for the visit. The purpose of the visit was to serve deficiencies that were observed during the annual inspection conducted on 6/20/25. Due to technical issues at the time of the annual visit, the LIC 809-D was not recorded, and deficiencies could not be formally issued. This report serves only to reflect the findings previously observed. No changes have been made to those findings and no new deficiencies are being added. The purpose of this report is to ensure that all observation from original visit are officially served to the licensee. Infants: LPA observed (1) infant in care. Napping equipment was observed, including (2) play yards and (1) crib and each equipped with a mattress and fitted sheet. Licensee did not have an updated 15-min sleep log which poses a potential health and safety risk to children in care. LPA discussed infant safe sleep with Facility Representative Marcela and Licensee via telephone. Facility shall supervise infants 0 \342\200\223 24 months while sleeping. Adhering to physical checks and documenting every (15-minute) check. Documentation of these checks shall be maintained in the infant\342\200\231s file and made available for Department review. Documentation shall include the date, infant\342\200\231s name, and the time of each (15-minute) check. Per Facility Representative Marcela all sheets are washed once a week and as needed. LPA provided Infant Safe for Family Child Care Homes from California -DSS-Manual -CCL Page 1 of 2 Children Files: LPA reviewed (2) children files for the following documentation: LIC 282 Affidavit Regarding Liability Insurance, LIC 700 Identification and Emergency Information, LIC 627 Consent for Emergency Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights, and LIC 9224 Acknowledgment of Receipt of Licensing Reports it was applicable. (1) of (2) children had the required documentation. C1 was missing proof of immunization and LIC 627 Consent for Emergency Medical Treatment which poses a potential health and safety risk to children in care. A notice of site visit was given and must remain posted for 30 days consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and Appeal Rights provided with Licensee Robin. Please see attached LIC 809-D for citations. (2) Type B was issued.
