What the inspector wrote

On 4/23/2026 at 1:30 pm Licensing Program Analysts (LPA)s, Stephanie Li and Mary Silva conducted an unannounced Proof of Correction (POC) inspection to ensure the deficiencies cited on 5/12/2025 during an Annual visit have been corrected. A risk assessment was conducted. Upon arrival, LPA met with Licensee Mary Silva. There were 0 children present and 2 adults. During the visit, Licensee stated she has completed the renewed mandated reporter training and that her daughter sent it to an email but she is unable to locate it. LPA M Silva spoke to licensee's daughter on the phone and requested the certificate be sent to LPA S Li's email address. Per licensee, assistant started training but has not finished it. Per licensee's daughter, she will help the assistant finalize the training and email both certificates to LPA S Li's email address. Licensee and spouse (assistant) are elderly and not tech savvy. LPA's reviewed requirement of completing mandated reporter training every 2 years. Based on observation and interviews, a technical advisory is being issued on page LIC9102. No deficiencies are being cited at this time. A notice of site visit was given to Licensee and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee Maria Castillo.
