Inspector’s narrative

What the inspector wrote

On 6/4/25 Licensing Program Analysts (LPAs) Portia Bowden and Javier Duran conducted an unannounced Required Annual Inspection at the above facility. At 9:30AM LPAs arrived to residence but were unable to make entry as there is a metal electric gate. LPAs called both numbers listed on LIS Facility profile sheet and left voicemails. At 9:45AM LPAs heard children inside the residence. LPAs text phone number in English and Spanish and called both numbers again. At 10:00AM licensee Sonia called back and stated she would arrive in 10 minutes. At 10:30AM Licensee arrived, LPAs provided entrance check list and conducted a tour of the home. Also present in the home were Licensee’s Husband and Assistant Doris. LPAs observed 6 children in care including 1 infant during inspection. Per Licensee there are 10 children enrolled. Per Licensee hours of operation are 6AM to 6PM Monday-Friday. Per Licensee she provides transportation to and from school. Per Licensee she does not provide overnight care. Per licensee she is apart of the food program and provides Breakfast, lunch, and PM Snack. Per Licensee outside food is not permitted in the facility. Per Licensee no children in care require medication or have food allergies. LPAs observed documents on licensing board (Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394), Child Care Facility Roster (LIC9040), Emergency Disaster Plan (LIC610a) and ER drill log last dated 2/25. LPAs observed Licensee’s current Mandated Reporter certificate exp 7/25. LPAs observed licensee’s current Pediatric CPR/First Aid Certification Exp 4/27. Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection: At 10:45AM LPAs observed a fully charged 2A10BC fire extinguisher with a receipt dated 9/12/24 in a kitchen drawer. At 10:50AM LPAs observed a functioning carbon monoxide detector on the ceiling wall and a smoke detector on the living room ceiling. The home was observed to have proper ventilation and heating for safety and comfort. LPAs observed a wall heater in the hallway next to the ½ bath children in care use, surrounded by a 4 ft mesh barricade that was bolted to the wall, making it inaccessible to children in care. The home maintains telephone service via cell phone and landline. Per licensee there are no firearms on the premises. Kitchen area was observed to be clean and free of clutter and have no sharp objects or chemical compounds accessible to children in care. At 11:00AM LPAs observed kitchen knives in a kitchen drawer, secured by a safety latch, inaccessible to children in care. At 11:05AM LPAs observed cleaning supplies in the kitchen cabinet underneath the kitchen sink, secured by a safety latch, inaccessible to children in care. Bathroom children use was observed to be clean and free of hazards. LPA observed bathroom sink cabinet to be secured by a safety latch Per Licensee she stores toilet paper in the cabinet. Per Licensee she does not provide potty training. At 11:10AM LPAs observed toys attached to a play yard as well as toys inside of the playyard with infant child. At 11:10AM Licensee removed all items, a technical violation will be issused. At 11:15AM LPAs observed age appropriate learning materials and toys in the daycare bedroom. LPAs observed 1 play yard as well as mats for other children in care. Per Licensee all linens are washed in the facility once a week and as needed. Outdoor Play for children in care is located in the enclosed backyard. At 11:15AM LPAs observed grassy area and age appropriate toys. LPAs observed no bodies of water on the premises. 3 children’s files were reviewed and observed to be complete. At 11:20AM LPAs observed a blank sleep log for 1 infant in care. Per licensee infant does not sleep while in care. LPAs advised Licensee to still document on the log. Licensee did not have a staff file for Assistant Doris Hernandez. Per Licensee Assistant has required immunizations but does not have a current Mandated Reporter Certificate or Current Pediatric CPR. LPAs advised Licensee to ensure anyone left alone with children in care has a current pediatric CPR. A type B deficiency will be issued. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, Licensee Sonia Velasco confirmed there are no Registered Sex Offenders living in the home. LPA completed the RSO profile in FAS. 1 Type B Deficiency was observed during today’s inspection for missing assistant file. 1 Technical Violation for toys attached to Playyard was observed. Exit interview conducted with Licensee Sonia Velasco, copy of this report, Appeal rights and- a notice of site visit were given and must remain posted for 30 days.
