What the inspector wrote

CASE MANAGEMENT INSPECTION CONDUCTED IN SPANISH Licensing Program Analysts (LPAs) Andrea Carter and Ashley Calderon conducted an unannounced case management visit for the purpose of confirming the removal of non-client adult resident Edgar Alonso. Upon arrival, LPA's met with Maria Alonso the Licensee, who gave LPA's a tour of the facility, including \342\200\234off limit\342\200\235 areas. The Licensee reported to the department she was in receipt of letter from Care Provider Management Branch dated December 18, 2025, indicating her employee may not work or be present in the facility due to criminal record exemption needed. LPA confirmed that Edgar Alonso was not present at the facility and has not resided in the home as of April 28, 2025, per the confirmation of removal signed by the licensee, received by the department on January 20, 2026. LPA Calderon reviewed the Family Child Home Addendum to Notification of Parents\342\200\231 Rights (LIC 995B) with the Licensee, and the licensee understands that Edgar Alonso is not permitted to be in the home at any time children are in care. The Licensee states she will have all parents of enrolled children sign LIC 995B and place in their individual files. During this visit the licensee completed and submitted a written declaration to the department stating to disassociate the non-client resident who is also her son and is no longer living in the home. LPA Carter will process the disassociation upon return to the regional office. The licensee understands the individual will need to receive an exemption granted prior to returning to the licensed facility when children are in care. No deficiencies will be cited today (date). A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted with Maria Alonso, the Licensee. Appeal Rights were provided.
