What the inspector wrote

On June 5, 2025, at 9:20AM, Licensing Program Analyst (LPA) Andrea Carter conducted an unannounced Annual Random inspection and met with Facility Representative (FR) Sheila Sangani. LPA disclosed the purpose of the inspection and was granted entry unto the facility. LPA was taken on a guided tour of both the indoors and outdoors of the facility. Teacher-child ratios were observed, and staff names recorded. All center staff that were present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. The sign-in and out sheets were reviewed. The Preschool program is one of three licenses on the premises. There is an lnfant program under license 198015919 and a school-age program under license 198010441. The hours of operation are 6:00 AM – 6:00 PM, Monday - Friday. Facility is currently licensed for a capacity of 112. At the initial start of the site visit, there is a total census of 64 children and 7 teachers present. The facility is made up of 4 classrooms. Room 2, 2-year old class: had 16 children and 2 staff, Room 4, 3-year old class: had 17 children and 2 staff, Room 3, 4-year old class: had 19 children and 2 staff, Room 5, 5-year old class: 12 children with 1 staff present at time of inspection. LPA observed all posting requirements for operation on the posting board and the following documents were observed: LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Report Continues Page 1 of 4 REPORT CONTINUES PAGE 2 OF 4 Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. The restrooms are located inside the classrooms, observed by LPA. Rooms 3,4 and 5 had 2 toilets and 2 sinks. Room 2 had 3 toilets, 2 sinks, and a nearby changing table within arm’s reach of a sink. LPA observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed red backpacks with first aid supplies kept in each classroom. Food menus were reviewed at time of inspection. Facility provides breakfast, lunch, and snacks. Food preparation area was toured and found in compliance for safety, cleanliness, and proper storage of food items. Drinking water is available in each classroom via water pitchers and water fountains. LPA confirmed Carbon monoxide and smoke detectors were present and determined to be in operable condition. Fire extinguishers were serviced on 03/10/25, observed by LPA. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. A review of medication policy, including administering, labeling, storage, and records was made. Currently there are no children enrolled needing medication administered. Allergy lists for children in care is posted in the kitchen and classrooms visible to all staff. Children with allergies are provided alternative foods. Fire & Earthquake drill observed conducted monthly with last drill conducted on 05/16/25 per observation. At 9:55 AM, outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. LPA observed a play structure with 2 plastic slide, and other age-appropriate toys around the play yard. Areas around or under climbing equipment, are cushioned with rubber mats that absorbs falls. Required shade and fencing were inspected. LPA advised that the children need to be within the direct care and visual supervision by the teachers at all times. The children have access to drinking water via water fountain or water pitcher and disposable cups, observed by LPA. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection. At 10:15 AM, staff and children’s records were reviewed for completeness. Inspection of required licensing forms were made and documented on the LIC 857 for children and the LIC 859 for staff. At least one center staff member has CPR and First Aid Certification. LPA observed current CPR and First Aid Certification for center Staff#1 is valid until 02/20/25. Role and responsibilities of being a Mandated Reporter were reviewed. 2 of 8 Staff present did not have proof of the required AB 1207 Mandated Reporter Training certificate on file. REPORT CONTINUES PAGE 3 OF 4 During the inspection, children present was observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment. The following were discussed: LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov . Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). REPORT END PAGE 4 OF 4 LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
