Inspector’s narrative

What the inspector wrote

LPA did observe a security guard outside the facility front entrance checking the two emergency exit doors that are at each end of the front of the facility. Director gave LPA a copy of the work order and bill for the parts needed for the front door to be fixed. LPA also observed that at the front desk in the lobby to the facility where the door is not working properly is the receptionist who is stationed there the entire day and who can monitor who comes in and out of the facility. Based on LPA observation, photographs taken, interviews conducted and documents reviewed and received, the preponderance of evidence standard has been met, therefore, the above allegation is found to be substantiated. California Code of Regulations (Title 22 Division & Chapter), is being cited on the attached deficiencies page LIC 9099D. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director, Vicky Gutzwiller . Page 2 of 2
