What the inspector wrote

This is an amended report to update language in report. Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced annual random inspection to the above facility on 09/19/2025. LPA arrived at the facility at 8:45AM, identified self and met with Laura Sydney, Director and Dr. Farah Khaleghi, Executive Director, who guided analyst on a tour of the facility. LPA provided the Director with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 2 classrooms; Room B with 2 children and 2 staff, Room C with 3 children and 1 staff. Due to low enrollment, Classroom A and D are currently not in use. This preschool follows Title 22 and Title 5 regulations. Facility operation hours are Monday through Friday from 7:00 AM to 6:00 PM. All areas identified on the Facility Sketch were inspected. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubbies to store their belongings. linens are taken home each week to be washed. Napping equipment (mats) were observed in a separate storage room. Per director, the isolation area is in the office. Age-appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.Director states that poisons are locked in the kitchen in a locked storage room that is inaccessible to children. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 02/12/2025, as indicated on service tag. The Dual Smoke and carbon monoxide detector was tested and are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. LPA observed the outdoors play yard garbage bin without a tight-fitting lid. In the kitchen, storage containers for solid waste, including movable bins, shall have tight-fitting covers that are kept on, and in good repair. In the kitchen, Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children\342\200\231s Records were reviewed. The name, address, and telephone number of child\342\200\231s authorized representative are on file. The inspection of required forms was made and documented on LIC 857. LPA also reviewed staff records. The review of Staff records was documented on LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB clearance, Current Influenza or declination, MMR (mumps, measles, rubeola), and TDAP. All staff have been given on-the-job training, sanitation principles, housekeeping, including universal health precautions. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 08/29/2025. Menus are posted one week in advance where they are visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides Breakfast, Lunch, and PM snack. During Kitchen inspection, LPA advised Director to remove the carrots dated best if used by 09/04/25 (todays date 09/19/25). First Aid supplies were observed in the classroom in a kit. According to the Director, medication is only administered to a child when accompanied by a doctor's note and is stored in the office. For additional information regarding the inspection and its tools and methods, please visit the Program website at www.cdss.ca.gov/inforesources/community-care-licensing/inspection-process .The director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 ( PIN 21-21.1- CCP ). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Lead testing was completed on 12/13/2022 with no lead exceedances or corrections needed.LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information . Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Site Supervisor Laura Sydney.The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). Licensee/Director was provided with a copy of the LIC 9224 Acknowledgement of Receipt of Licensing Reports during this visit. Report amended to update language in report. Fixtures, Furniture, Equipment and Supplies CCR 101239(f)(1)(1) All containers used for storage of solid waste, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.
