Follow-up on corrections·April 3, 2023

Licensing Program Analyst (LPA) Seung Lee conducted an unannounced case management inspection. Upon arrival LPA Lee met with assistant Director Susan Situ. It was observed that 02/22/2023 a parent of a child in care (Child#1) brought up concerns to this facility regarding what her child told her happened at the facility. File review for this child shows that Child#1's last day was 02/23/2023. The facility stated that care for Child#1 was terminated based on the well being of the staff members that work at the facility. The facility stated that the staff members were concerns due to being accused of doing something they did not do. During the inspection LPA Lee reviewed the termination conditions in the admissions agreement of the facility. LPA Lee did not observe any of the conditions that was applicable to Child#1's service being terminated on 02/23/2023. The facility was advised during this inspection that parent's have the right to bring up concerns with the facility since it is within their rights as parents. Therefore, the facility is being cited on this date for not complying with regulations regarding parental rights. Please see 809D page for details. The notice of site inspection must remain posted for a period of 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars.
