What the inspector wrote

On 02/27/2024 at 2:30pm, Licensing Program Analysts (LPAs), Crystal Green and Shushanik Safaryan conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPA met with Director, Erica Patterson. This facility is currently licensed as an Infant program with toddler option component. The Infant program has a capacity of 8 serving ages 0-24 months and the toddler component has a capacity of 10 serving ages 18 to 24 months. The Licensee, Crescenta Valley United Methodist Church, is requesting to remove the toddler component and increase the overall infant capacity to 28 children serving ages 0- 24 months. Hours of operation will be Monday to Friday from 7:30am to 6pm. This facility is located on the premises of Crescenta Valley United Methodist Church. The building was observed to be 2 floors. Infant program operates out of the first floor of the building. There is also a preschool program (191200283) located on the same premises which was observed to operate separately both indoors and outdoors. Present during today’s inspection were 10 children with 4 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. Licensing staff informed Erica Patterson that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. Licensing staff continued to tour the facility and measured new additional indoor and existing outdoor activity space. This Infant program will operate out of 3 classrooms: Bunnies (ages 0-12 months), Ducklings and Piglets’ (12- 24 months). LPA Green obtained measurements for the new additional classroom Piglets’. The Piglets room measured for 585.60 square feet which can accommodate the requested capacity increase of 10 additional infants. LPA Green did not obtained measurements for the existing licensed classrooms. Page 2: Licensing staff observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping mats, bookshelves, and other activity supplies for the children. Drinking water will be available in the classrooms via filter water jug. LPAs observed a separate napping area located within Bunnies classroom which contained 8 cribs. LPAs observed a changing table located within each classroom accessible within arm’s reach of a sink. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 1/31/2024. LPAs observed a total of 5 sinks and 5 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 28 children. LPAs observed the restrooms located within Ducklings and Piglets to be inaccessible to the children in care. Per Director, Erica Patterson, the infant program does not offer potty training to the children in care which is outlined in their parent handbook. LPA Green advised Director to make one of the restrooms accessible to children in the event a child requests to utilize a toilet. The facility currently has a fully fenced playground area. Licensing staff observed the infant outdoor area to be separated from other programs using a wrought iron fencing which was observed to be at least four feet high. The total square footage for all the outdoor activity space is 1645 square footage, which can accommodate 21 children at one time, insufficient for the number of children being requested. LPA Green observed a usable yard space next to the existing infant play yard that is not being used by either the Infant or Preschool Program. LPA Green consulted with the director regarding relocating the fence an additional 20 feet outward in order to increase designated infant yard. Based on LPA measurements the increase will add an additional 860 square feet. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via filter water in the child’s personal water bottle. LPAs observed all hazardous items on the playground to be inaccessible to children. Director, Erica Patterson, was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Page 3: The following corrections are needed prior to the issuance of the capacity increase request: • The applicant will need to submit proof of relocating the fence to create additional outdoor space for the Infants. Licensee Representative, Erica Patterson, understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. After receipt of the proof of corrections and final file review is conducted a license will be granted for a capacity increase to 28 children. Exit interview conducted and report was reviewed with the Licensee Representative, Erica Patterson. A notice of site visit was given to the facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
