What the inspector wrote

On January 20, 2023 at 11:35 AM, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced required 1-year inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Director, Darlene Eliasvaca who guided the LPA on a tour of the facility. This is a preschool age which operates Monday through Friday from 7:30AM to 6:00PM. Per the assistant director there are 84 children enrolled. The following classrooms were inspected: Room 1: 10 children with 2 staff, Room 2: 8 children with 2 staff, Room 3: 6 children with 1 staff, Room 4: 11 children with 2 staff, Room 5: 15 children with 2 staff, Room 6: 11 children with 1 staff, Room 7: Art room, Room 8: 9 children with 1 staff. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff present have been fingerprint cleared and associated to the facility. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license. The classroom furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. LPA observed in classroom #4 that the sinks are not in good repair; sinks are loose and are being held by plastic material (images obtained). LPA observed classroom carpet in room #1 being dirty from mud (rain). Per assistant director the carpets have become dirty due to rain. The facility provides snacks and water for children in care. Parents provide all meals for children in care. Food is stored in day care children\342\200\231s cubbies. LPA observed that there are functioning smoke and carbon detectors in the facility. LPA observed a fully charged and serviced fire extinguisher. Emergency disaster drill was last conducted on 10/18/2022. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. LPA observed cabinets beneath the sink that store cleaning compounds. These cabinets are not made inaccessible to children in care. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPAs reviewed staff records, staff records were complete. Children\342\200\231s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. SB792 (Immunization Requirements for Staff and Employees) was discussed with the Director. The Director and staff do currently have immunization documentation. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Assistant director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Based on the LPA\342\200\231s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety. An exit interview was conducted, and a copy of the report was provided to director along with appeal rights and Notice of Site Visit.
