What the inspector wrote

On June 3, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. The purpose of this inspection is to follow up on an incident that occurred on 05/08/2025 and was reported to the department in a timely manner on 05/09/2025. LPA was greeted by Assistant Director, Darlene Eliasvaca who guided LPA on a tour of the facility. LPA observed 82 children in care. LPA observed the area where the incident occurred and reviewed the climbing structures safety rules. Per assistant director, Child #1 (C1) was swinging on the one of the handrails on the climbing structure. While swinging C1 slipped and fell on the padded floor under the climbing structure. LPA review climbing structures safety rules, and did not observe anything stating no swinging. LPA informed assistant director that the climbing structure's handrails should be used for their only their purpose (holding onto while climbing). C1 did not receive any medical attention regarding the slip/fall. There are no deficiencies being cited at this time as the incident appeared to be an accident the occurred very quickly and was unable to be prevented. An exit interview was conducted and a copy of this report was provided along with Appeal Rights. A Notice of Site Visit was provided and must be posted for 30 days.
