What the inspector wrote

Prior to entrance to the facility, LPA Mayra Rivera conducted a Covid 19 assessment and based on the responses to the facility assessment questions, LPA Rivera determined safe to proceed. On Wednesday , August 19, 2021 at 10:00 AM , Licensing Program Analyst (LPA) Mayra Rivera conducted a continuation capacity increase inspection and met with Supervisor Zayra Rodriguez who guided LPA Rivera on a tour of the facility At 10:05 AM, LPA Rivera entered Room #1 to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera observed 3 children. The following staff were present during this visit: Staff # 1, Staff # 2 , # 3 and teacher child ratio were observed. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children, inaccessible to children inside bottom sink with a tight fit closure and recommended to add a safety latch. LPA observed the furniture and equipment in good condition, age appropriate and free of sharp, loose, or pointed parts. LPA observed hand washing sink area safe, sanitary and operating properly and floors clean and safe. At 10:15 AM, LPA Rivera entered Room # 2 to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera observed 3 children. The following staff were present during this visit: Staff # 4, Staff # 5 and, # 6 and teacher child ratio were observed. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children, inaccessible to children inside bottom sink with a tight fit closure and recommended to add a safety latch. LPA observed the furniture and equipment in good condition, age appropriate and free of sharp, loose, or pointed parts. LPA observed hand washing sink area safe, sanitary and operating properly and floors clean and safe. At 10:26 AM, LPA Rivera entered Room # 3 to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera did not observe children nor staff. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children, inaccessible to children inside bottom sink with a tight fit closure and recommended to add a safety latch. LPA observed the furniture and equipment in good condition, age appropriate and free of sharp, loose, or pointed parts. LPA observed hand washing sink area safe, sanitary and operating properly and floors clean and safe. At 10:31 AM, LPA Rivera entered the restroom located between room #1 and # 2 and observed one sink and two toilets. The facility currently has a waiver for one sink to be shared between classrooms # 1 and #2. Restroom for room # 3 is located in the hallway across from room # 3. The restroom has one sink and one toilet and the children will use a stepper because the toilet is standard size. LPA observed the restroom to be in good condition. LPA observed inside each classroom trash cans to discard food and waste with tight fitting lids to be in good repair. Drinking water is readily available both indoors and outdoors. LPA observed water jugs and disposable cups inside each classroom and outdoor play area. Los Angeles Unified School District (LAUSD) provides breakfast, lunch, and snack. For ventilation LPA observed central AC/heater and room temperature at 69 degrees. LPA observed the sign in sheets for classroom #1 and #2. For isolation, children will utilize the restroom and isolate in the nurse's office. The facility currently has no children with or on medication. Children will not be napping due to the program being part-time. Smoke detectors are present in the facility, but were not tested as they are hardwired through the building. LPA observed the required 2A10BC fire extinguisher located in the classroom #4 and the valve on the green area indicating fully charged and serviced on May 3, 2021. LPA observed first aid kits inside the classrooms and on the bulletin boards the earthquake and fire drill log and licensing forms LIC 308 Designation of facility responsibility, LIC 613A Personal Rights, LIC 610 Emergency Disaster Plan, License, PUB 293 Parent's Rights, PUB 269 Child Passenger restraint system, Daily Schedule, and menus. At 10:45 AM, LPA Rivera inspected the outdoor area for safety, comfort and cleanliness. For outdoor play area the children will be utilizing the back space behind classroom #1 and # 2..The facility was observed to be free of flies, insects and rodents .LPA observed the outdoor playground equipment in safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed a shed and main entrance gate locked with a key pad lock. LPA reviewed children\342\200\231s records and issued Children's Record Review form (LIC 857) to supervisor Zayra during this inspection. Staff files were not reviewed on this date as they are housed at headquarters Human Services Association. LPA reviewed supervisor Zayra pediatric First Aid/CPR certification. Certification expires 4/27/23. The following was discussed with the Director: Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. LPA informed site supervisor Zayra that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. At this time, the facility is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today. Based on measurements taken during June 21, 2021 inspection and no added encumbered space; upon approval the facility shall be licensed for a capacity of 45 children. LPA Rivera provided technical assistance to place a safety latch on the sink cabinets. LPA provided The Notice of Site Visit (LIC 9213) and explained that it must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Zayra Rodriguez. LPA explained and discussed this report, including the appeal rights and provided a copy. The signature on this report acknowledges and confirms receipt of these documents.
