What the inspector wrote

Licensing Program Analyst (LPA) Elka Chavez conducted an unannounced required 1-year inspection. At 10: 20 AM LPA met with lead teacher, Yoselin Gutierrez and was joined by Child Development Coordinator, Leticia Martinez, who guided LPA on a tour of the facility. This program operates on the grounds of Miles Elementary School in rooms number 1, 2 and 3. There are two sessions 8:00 AM - 11:00 AM and 12:00 PM - 3:00 PM, Monday through Friday. At 10:30 AM all areas identified on the facility sketch were inspected. Upon arrival, LPA observed 11 children with 3 staff in room #1. LPA did not observe any children or staff in room #2 and #3. Teacher child ratios were observed to be in accordance with Title 22 regulations. The facility is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. The following was observed during tour of facility: Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. At 10:55 AM LPA observed desk, tables, chairs and boxes being stored in classroom #3. LPA observed no staff or children in classrooms #2 and #3. Leticia Martinez stated that the classrooms are not being used due to low enrollment. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the single restroom next to room #2 is used as the designated ill isolation bathroom. Parents are contacted immediately when children are determined to be ill. The facility currently has no children with or on medication. Children will not be napping due to the program being part-time. At 11:15 AM LPA did not observe menus posted. Per staff #1 the school district has not been providing copies of the menus due to a data breach in the LAUSD system. Morning and afternoon meals are provided by Los Angeles Unified School District (LAUSD). At 11:20 AM LPA observed the kitchen, preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. LPA observed disinfectants, cleaning solutions and other items that are dangerous to children, accessible to children under the sink inside classroom #1. LPA observed the items were moved into the storage room located inside the classroom. LPA observed the door to the storage room is kept closed and locked. Smoke detectors are present in the facility but were not tested as they are hardwired throughout the building. At 11:45 AM LPA observed the required 2A10BC fire extinguisher located in classroom #4 and the valve on the green area indicating fully charged and serviced on November of 2021. Outdoor activity space equipment and surface is maintained in a safe condition as is free of hazards. LPA did not observe climbing equipment and slides. LPA did not observe any bodies of water on the premises during this visit. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. At 12:30 PM children\342\200\231s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Staff files were not reviewed on this date as they are housed at the Human Services Association headquarters in Bell Gardens. LPA did not observe staff #3 associated to the facility. At 1:30 PM LPA was provided with a copy of the licensing forms that were sent via fax to the regional office for processing on 8/18/21 at 4:51 PM. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative, Leticia Martinez was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. There were no deficiencies cited during today's visit in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1 A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the facility representative, Leticia Martinez.
