Licensing Program Analyst (LPA) T. Tran conducted an unannounced Case Management Incident visit at Kirkwood Christian Schools to follow up on a self-reported incident occurred on 06/24/2025. The Monterey Park Southwest Office received the writing report on 06/24/2025. Upon arrival, LPA met with LuAnn Irvine, Office Manager, who provided LPA with a tour of the facility. LPA children were napping. No concerns with napping ratio. LPA completed staff and child records review. Interviews were conducted with staff, child, and parent. LPA obtained child's document and personnel report. LPA took photo of the play structure for the record. Based on the interviews conducted. On the day of the incident, there were 18 children with two staff present. According to the available information, this incident took place at the playground, during outdoor play, the child missed a step while climbing up a play structure. Child slipped and hit the right eyebrow on the structure. The facility informed parent in a timely manner. Child was taken to the doctor and received superficial glue to close the wound area. Child returned to school the next day without any restriction. No further action is required for this incident. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, LuAnn Irvine.
