What the inspector wrote

Licensing Program Analyst (LPA), T. Tran arrived at the above licensed facility for the purpose of concluding the investigation of the above complaint allegation. Upon arrival, LPA met with Superintendent, Jennifer Hartl. Based upon the evidence obtained during the course of the investigation through interviews and record reviews. 12/25/25, around 1:22PM, during naptime staff confirmed that C1 (see LIC 811) likes to cover the head when napping and staff failed to remove the blanket by allowing the child napping with a blanket covered the head until the parent arrived early and observed such incident. Based on the facts presented, this incident was the result of Title 22 Personal Rights Violation, therefore, the above personal rights allegation that staff did not provide a safe environment for children while napping was substantiated. A finding means that the complaint is substantiated, and the allegation is valid because the preponderance of the evidence standard has been met. Facility was cited type B deficiency. Please see Complaint Investigation Report LIC 9099D for deficiency cited. Exit interview conducted and report was reviewed with the facility representative, Jennifer Hartl.
