ANNUAL /RANDOM INSPECTION CONDUCTED On April 21, 2026, at 11:10 AM, Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Annual / Random Inspection and met with licensee Maria Andrade . LPA disclosed the purpose of the inspection and was granted entry into the facility. LPA was taken on a guided tour of the facility and provided licensee with a copy of the Facility Entrance Checklist. Adults in the home were discussed, and all have criminal record clearance. There were no children present during today’s inspection. Currently, the licensee provides after school care. There are 7 children enrolled. The children's roster was reviewed by LPA and is current. The facility’s hours of operation are Monday through Friday from 6 AM to 6 PM. This is a single story home which consists of (4) bedrooms, (3) bathrooms, and detached garage. Care is mainly provided in the children's playrooms adjacent to the kitchen. Areas accessible to children are: living room /dining area, day care rooms, and bathroom across from the laundry room. Areas off-limits to children and parents are: (2) bedrooms , (2) bathrooms, and detached garage. LPA observed the off-limit doors to be locked. The licensee acknowledged that children may never enter these off-limit areas. All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. The facility has central air and heating and maintains telephone service via landline and cell phone. There is a screened fireplace in the living room observed by LPA. Facility License, Emergency Disaster Plan, and Parents’ Rights were posted and observed at time of inspection. Disaster drill log was also available during today’s inspection, with last disaster drill conducted on 04/03/26 . LPA observed the required 2A10BC fire extinguisher with service tag dated: 01/06/26. Dual smoke and carbon monoxide detectors tested and in operable condition at time of inspection. First Aid kit is located in the hallway closet, observed by LPA. LPA observed main care area has age-appropriate toys, free of loose and sharp parts. LPA observed napping mats against the wall, not blocking entrances or exits. A changing table, highchairs, and child size tables and chairs. observed by LPA. The bathroom available for children in care was observed to have an operable toilet/sink and adequate toileting supplies for children in care. There is a safety lock on the cabinet under the sink. No hazards observed. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible stored in off-limit areas of the home. The Licensee states that there are no poisons in the home and understands that poisons must be locked. Per the licensee, children have access to the backyard for outdoor play. LPA observed yard has grass and adequate perimeter fencing through-out the property. LPA observed a play structure with a slide and swings, riding toys, a playhouse, and other age-appropriate toys. LPA did not observe any objects that could be hazardous to children in care. Per licensee there are 3 dogs on the premises. 2 dogs are kept in a locked gated area on the side of the home, inaccessible to children in care. 1 dog is kept in an off-limits bedroom inside the home. Per the licensee, supervision is always provided during outdoor play. Food is provided for children in care. Licensee was reminded that any food brought from the children’s homes shall be labeled with child’s name and properly stored. Per the Licensee, there are no children enrolled that requires medication administered. Infant Care: Currently, there are no infants enrolled. The Licensee was advised; infants shall not be swaddled in care. Car seats shall only be used for transportation purposes and shall not be used for sleeping. LPA discussed the Safe sleep regulation, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0 -24 months. — Children’s records were reviewed for: (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months). All required licensing forms are in file. Staff records were reviewed for: approved Pediatric First Aid & CPR certification, Criminal Record Statement, Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. —CPR Card valid until: 01/16/28 —Mandated Reporter AB1207 valid until: 4/28/27 The following was discussed: · Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers, or any other item that falls into that category are permitted in the facility. Effective January 1, 2010, Licensees of family childcare homes are required to ensure that at least one staff member with current training in Pediatric First Aid & CPR is on site at all times when children are present. · Reporting Requirements: Changes should be reported to the Department as soon as they occur, such as construction, remodeling, telephone number changes and/or if you move from your home. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. Per the licensee, there are 3 pet dogs in the home that do not interact with day care children. The Licensee confirms there are no smokers in the home, there are no weapons or firearms in the home, and there are no pools, spa or other bodies of water on the premises. ..............................................................Report Continues 4 of 5 Pages............................................... Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Maria Andrade, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
