What the inspector wrote
On 12/02/25 at 12:20pm, Licensing Program Analyst (LPA) Diana Ortiz conducted an unannounced Annual random inspection to the above facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee, Martha Vazquez, who guided the LPA on a tour of the facility. Adults in the home were discussed, and all have criminal record clearance. There were two (2) childcare children present during today’s inspection. Licensee states that there are currently nine (9) children enrolled. Per licensee, the facility’s hours of operation are Monday through Sunday from 6am to 5am. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Facility capacity is in compliance for a small Family Child Care Home. LIC 126 was provided.
This is a single-story home with a second home in the lot. The second home in the premises has a separate address 144 N. Vernon Ave, Azusa CA 91702, per licensee home is empty and used as a storage place. The home consists of 3 bedrooms, 3 bathrooms, kitchen, living room, dining room, family room/daycare room, fenced front yard, side yard, fenced back yard and in ground swimming pool. Currently residing in the home are three adults (criminal record clearance on file) and no minors. There was a total of two (2) adults present during this inspection, licensee, and licensees assistant.
At 12:35pm, Licensee guided analyst on a tour of the facility.
Per licensee areas off limits to children and parents include:
3 bedrooms, 2 bathrooms, kitchen, dining room, living room, front yard fenced back yard, swimming pool located in the back yard and second home located in the premises adjacent to the side yard.
Per licensee areas accessible to children are
: Family room/daycare, 1 bathroom located in the daycare room and the side front yard. Per licensee, off limit areas are also locked during operating hours, LPA observed doors to be locked.

LPAs observed the following required posted documentation in the main entry way of the facility: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. LPAs reviewed facility records for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan.
The Licensee uses the side yard for outdoor play. The outdoor play area was observed to be fenced. Licensing staff observed the outdoor yard has toys and other materials for children to play with. LPA observed bodies of water during this visit via in-ground swimming pool. LPA observed an iron rod gate surrounding the pool. The gate was observed to self-close and is 5 feet tall and has a maximum vertical clearance of two inches from the ground to the bottom of the enclosure. The swimming pool has a self- latching gate which opens away from the swimming pool, the gate also has a key lock with key placed no lower than 60 inches above the ground. LPA observed a life ring with a minimum diameter of 17 inches
and is labeled as approved by the United States Coast Guard. LPA also observed a rescue pole with a body hook and minimum fixed length of 12 feet. LPA observed a pool alarm. The pool alarm was tested during today’s visit and observed to be in working condition. Licensee provided a daily inspection log to ensure pool safety. The swimming pool is in compliance with Title 22 regulations. Per Licensee the children do not use the swimming pool during the hours of operation. There is a door which leads to the back yard, the door always remains locked, and it has a second lock on the top.
LPA observed two (2) dogs during today’s visit. Per Licensee, the dogs are kept outside on the side yard/dog run during Daycare hours making them inaccessible to children.
Per licensee, they provide lunch, PM snack and dinner for children in care. Sick/isolation area is in the daycare room. Per Licensee, if the children were to feel ill, they will wait in the isolation area which is located on the corner of the daycare room for parents to pick them up.
LPAs did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other items that fall into these categories are not permitted in a family childcare facility. LPAs observed age-appropriate toys free of loose and sharp parts available for the children in the front fenced yard and all trees, shrubs, and plants are maintained.

Licensee file was reviewed for the following: LIC 508- Criminal Record Statement, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Pediatric First Aid and CPR certification expires on 12/03/2025 for licensee. Mandated Reporter Training for licensee was on file during todays visit and expired on 08/03/25.

Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Poisons are locked in the off-limits shed in the backyard. LPA advised that any poisons should be locked under key or combination lock. Licensee does not have firearms in the home.
The valve on the required 2A 10BC fire extinguisher indicates fully charged and was purchased on 12/02/25. Licensee provided LPA with purchase receipt. Smoke and carbon monoxide detectors were tested and were operable on this day.
All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Per Licensee, last fire drill conducted was on 08/2025.
The home is observed to be clean and orderly. There are first aid supplies available. There are age-appropriate toys available for children. Appropriate sleeping arrangements in form of playpens and mats were observed.
Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Licensee states she is currently caring for one (1) infant. LPA observed a sleep log and LIC 9227 for infant.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, the Licensee Martha Vazquez, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on Licensing staff observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
A notice of site visit was given and must remain posted for 30 days.
