What the inspector wrote

On April 4, 2026, at 12:15 pm Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced annual inspection at the above facility. LPA was greeted by Licensee, Lydia Crosby who guided LPA on a tour of the facility. Per licensee there are 8 children enrolled and LPA observed 1 infant and 3 preschool aged children in care. Hours of operation are Monday \342\200\223 Friday from 7:30 am \342\200\223 5:30 pm. Family members residing in the home is 1 adult who obtains criminal record clearance. This is a single-story home that consists of 3 bedrooms, 2 bathrooms, living room, dining area, kitchen, front yard and backyard (fenced). Off-limit areas include 2 bedrooms, 1 bathroom and back yard (fenced). Areas accessible to children in care are living room (main day care area), 1 bathroom in the hallway, kitchen and front yard. At 12:30 pm, licensee guided LPA on a tour of the facility. The daycare is located at the back house of the property. Per licensee, parents enter through driveway gate and walk to the back house. LPA observed the front yard where children use for outdoor play. LPA observed several age-appropriate toys for children in care. Upon entrance to the home, LPA inspected the living room area which is the main daycare area. LPA observed several age-appropriate toys for children in care. Napping equipment in the form of foldable cots and play pen was also observed in day care area. Bedding was observed to be clean, orderly, and stored properly. Per licensee, bedding is washed at the facility weekly and children take home their blankets every Friday for washing purposes. Page 1 of 4 Licensee then lead LPA through a door in the living room area which leads to a small hallway to the bathroom and 1 bedroom that is accessible to children in care. The bathroom was observed to be clean and orderly. In the same hallway LPA observed a small dog that was away from the daycare children. LPA reminded licensee, if they dog will be around the day care children it is best practice to provide 100% supervision. Bedroom 1 was then inspected, per licensee children do not usually enter this bedroom but they do have access to it. LPA was then guided to the dining and kitchen area, were LPA observed a table and chairs. LPA did not observe any hazardous materials that can pose a danger to children in any areas that are accessible to children in care. All areas were observed to be clean, orderly, comfortable, and well ventilated. There are first aid supplies available. Per licensee, the facility does provide all meals for children in care. LPA reminded the licensee that food brought from home must be labeled with the child\342\200\231s name, stored properly and or refrigerated. If a child becomes ill, the child is isolated in bedroom 1 while the parents are called. A cot is available if needed. Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children. LPA observed a functioning smoke and carbon monoxide detector in the living room. LPA observed a 2A-10BC fire extinguisher anchored in the kitchen. The valve on the fire extinguisher was pointing to the fully charged green section and was serviced on 02/23/2026. There is a working telephone on site. Per Title 22 Emergency Disaster Drill must be conducted at least every 6 months. LPA observed the last Emergency Disaster Drill documented was on 2/18/2026. Per the licensee, there are no weapons or bodies of water on the premises. Licensee does obtain current Pediatric First Aid and CPR certification which expires on 2/21/2028. LPA conducted a children file review and deemed not complete. Licensee has not been documenting C1\342\200\231s 15-minute safe sleep log for LPA to review. Per licensee, they always observe the infants while they are napping. LPA reminded licensee they were previously cited on 4/03/25 for not documenting 15 minute safe sleep and LPA did provide safe sleep material for licensee\342\200\231s reference during that visit. LPA provided a safe sleep log sample for licensee to use. Facility will receive civil penalties for a repeated citation within one year. LPA also encouraged licensee to read Safe Sleep PIN. Page 2 of 4 LPA conducted staff file review and deemed not complete. S1 did not obtain current childcare mandated reporter training certificate. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years at www.mandatedreporterca.com Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394) and Emergency Disaster Plan (LIC610a). The licensee is observed to be operating within the license capacity limitations. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Page 3 of 4 During the exit interview, licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on the LPA\342\200\231s observations and record review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety. A notice of site visit was given and must remain posted for 30 days.
