Correction check·June 3, 2026

On June 3, 2026, at 2:00 p.m. Licensing Program Analyst (LPA) Peter Bishop arrived at the above facility for the purpose of a Case Management - Plan of Correction (POC) Inspection to follow up on a Type B Deficiency cited on April 24, 2025 for failure to have a valid Safe Sleep Log during an Annual Inspection. LPA Bishop announced purpose of inspection and was granted entry to the home by Licensee Consuelo Cota. LPA took a census and there were 3 children present at the time of this visit. The purpose of today’s visit is to verify that a valid safe sleep log is being kept for infants. LPA Bishop was able to verify that there is a current valid safe sleep chart being kept for the one infant in care at the present time. Licensee was reminded to make sure that there is always a safe sleep log being kept for all infants in care for their safety. There will be no deficiencies cited today June 3, 2026. A Clearance Memo will be issued today as proof that the deficiency is cleared. A copy of this report was reviewed and given to Licensee Consuelo Cota. Page 1 of 1
