Case management incident: Child wandered away from supervised preschool playground on 6/24/25 and was found unsupervised in school lobby approximately 5 minutes later. Video footage documented incident. Two Type A citations issued for failure to provide continuous supervision.

LPA conducted an annual inspection of the state preschool program and reviewed classrooms, outdoor space, and records. The visit identified tripping hazards from exposed tree roots and the need to resolve the use of additional rooms and a staggered outdoor schedule under the license.
