What the inspector wrote

On 3/14/2022 at 8:34am, Licensing Program Analyst (LPA) Jennifer Hua conducted an unannounced Required - 1 year inspection. Upon arrival, LPA met with assistant director, Mayra Mondragon. Assistant Director stated that director should be here around 10. A COVID-19 risk assessment was conducted prior to entry to the facility. LPA provided Assistant Director with a copy of the LIC 125 Entrance Checklist to facilitate the inspection. This is an Infant Program. Facility is also licensed Preschool # 198012765 and for School-Age #198012766. Business hours are from Monday to Friday, 6:30 AM to 6:30 PM. Director Ana Granados arrived at 10:08am. LPA was guided to a facility tour by assistant director. All areas identified on the Facility Sketch were inspected. There are 2 classrooms for the infant program: Classroom- Toddler, room was empty during the tour. Classroom - Infant - there were 5 infants with 2 staff The facility was observed to be within the license capacity and limitations. Children's roster was reviewed and is not current. Sign-In and Sign-Out sheets were reviewed. Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for infants\342\200\231 belongings and napping equipment/cribs was inspected and meet all regulatory requirements. Availability of drinking water was reviewed. The children have their own sippy-cups with their names written on them. Age appropriate sinks inspected for availability, good repair, water temperature, area safety and sanitation. Changing table has raised sides at least three inches high and is within an arm\342\200\231s reach of a hand washing sink. Toys observed to be clean. . Page 1 of 3 Facility provides bedding materials for infants and is laundered at the end of each day. Storage for children's belongings was reviewed. Food preparation areas were toured for safety, cleanliness and proper equipment. Meals are provided by parents for the infants, and facility provides morning snack, afternoon snack and lunch for the Toddlers. Bottles, dishes, and containers were labeled with infants\342\200\231 names; dates were visible on all children's bottles and other refrigerated items. Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. No hazards or bodies of water observed. Teacher-infant ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of infants are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of infants were observed by LPA. Staff and Infant Records were reviewed for completeness. Inspection of required forms made. Staff and Children\342\200\231s Records were reviewed. Criminal Record Clearances were reviewed for adults. Pediatric First Aid/CPR reviewed and is current. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following was discussed with the Director: Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited. No infant walkers, no Johnny Jumpers, no excersaucers or any other item that falls into that category are allowed in facility. Page 2 of 3 The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department prior to such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Sudden Infant Death Syndrome (SIDS) and Never-Shake-a-Baby were discussed. A hard copy of A Child Care Provider\342\200\231s Guide to Safe Sleep was discussed. Director stated she has it. Mandatory Forms for the children\342\200\231s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed and observed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov . Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. LPA advised Director that all staff 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.
