Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA), T Tran arrived at the above licensed facility to conduct an unannounced subsequent complaint inspection for the purpose of concluding the investigation of the above allegations. Upon arrival, LPA met with Nancy Lopez, Education Program Manager. Based upon the evidence obtained during the course of the investigation through interviews, record reviews, and observation, the evidence does not support, nor disprove the above allegation of the facility failed to provide changing table therefore a child has been changed standing up. This is a preschool facility serving children ages 2-5 years old no changing table is not required. Furthermore, the allegation of staff provided inadequate supervision as a result a child in care being hitting other children in care. Record reviewed indicates the program had completed the referral process, parents and disabilities team were onboard to ensure child's need is met. In addition, child's schedule has been modified, and one-on-one support was provided. Therefore, the allegations have been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. The copy of this report was explained and issued to noted person.
