What the inspector wrote

Please see LIC 9099-D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted, a copy of this report, appeal rights were given to Director, Melissa Villareal.
