What the inspector wrote

Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Annual/Random inspection at the above facility on 04/21/26 at 02:11 p.m. At approx. 02:16 p.m., LPA met with licensee Susana Alonzo who guided LPA on a tour of the facility. There were 05 children present in the home. According to the licensee, 12 children are enrolled and children’s schedule varies. Operation hours are Monday – Friday 06:00 a.m. – 06:00 p.m. LPA conducted the inspection in Spanish. This is a one-story home which consists of 3 bedrooms, 2 bathrooms, living room, dining room, kitchen, back patio, garage (converted playroom), and backyard. According to the licensee, children use the living room, dining room, kitchen, bathroom (located between kitchen and back bedroom), garage, and backyard. Family members residing in the home are 2 adults and 0 minors. The licensee stated that their grandson is fingerprint cleared, goes to college, and comes back and forth to the facility. LPA provided consultation. According to the licensee, there are 2 dogs on the premises. LPA toured all areas identified on the facility sketch used by children during this visit and were inspected for safety, comfort, and cleanliness. Tour began in the living room where LPA observed furniture and 2 children napping on mats. Tour then continued to the dining room where LPA observed a dining room table with chairs and stored cots and mats. According to the licensee, older children use the dining room for lunch and snacks, and younger children eat in a child size table for lunch and snacks in the kitchen. Children’s bathroom is located between the kitchen and back bedroom. LPA observed that the children's bathroom is safe and in sanitary conditions. LPA observed off-limits areas inaccessible to children. At 02:22 p.m., smoke and carbon monoxide detectors were observed to be operable. The valve on the required 2A10BC fire extinguisher indicates fully charged and was serviced purchased on 06/09/25, per the attached service tag. The licensee has a first aid kit readily available. Page 1 of 3 LPA observed the following required posted documentation in the dining room: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form, LIC 610- Emergency Disaster Plan and PUB 269 California Child Passenger Safety Law. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill conducted was on 03/18/26. Children’s roster was available for review during the inspection. Per licensee, food is provided to children in care. Licensee was advised that if food is brought from the children’s homes, all containers must be labeled with the child’s name and properly stored or refrigerated. The facility has a working telephone, ventilation, and heating. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children. The licensee states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock. Per the licensee, there are no firearms, weapons or bodies of water in the home. According to the licensee, children use the back yard for outdoor play. LPA observed adequate shade and age-appropriate play equipment. The back yard is completely fenced. Per the licensee, children use the garage for activities and observed age-appropriate toys and materials for children. LPA did not observe any objects that could be hazardous to children in care . LPA advised the licensee to check with the American Standard Testing Materials for Playground Guidelines and/or to use the manufactures recommendations for the playground equipment in efforts to be more in line with safety guidelines. Licensee understands that children should be supervised at all times. Children and staff records were reviewed and observed to be complete. Children and staff names were documented on the LIC 811, and a copy was provided to the licensee during the inspection. AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018, any person who works in a child care facility shall complete the training and renew the training every 2 years. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the licensee Susana Alonzo confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. At this time, the facility is in compliance with California Title 22 Regulations. Therefore, there are no deficiencies being issued today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Susana Alonzo.
