On 03/11/26 at 11:30am, Licensing Program Analyst (LPA) Diana Ortiz conducted an unannounced annual inspection to the above facility. LPA was greeted by Licensee, Clara Recendez to whom the reason for the visit was explained. Licensee guided LPA on a tour of the facility. Census was taken and LPA observed five (5) children in care. Also present was the Licensee\342\200\231s spouse, and Licensees assistant. Per Licensee, there are currently 14 children enrolled. A current children\342\200\231s roster was available for review. Hours of operation are from 6am to 6pm, Monday through Friday. The licensee was observed to be operating within the license capacity limitations at the time of inspection. LIC 126 Entrance Checklist was provided to Licensee. LPA observed the following required posted documentation in the main entry way of the facility: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. LPA reviewed facility records for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan. This is a one-story home which consists of 4 bedrooms, 2.5 bathrooms, living room, dining room, kitchen, den, garage, front and backyard (fenced). Currently residing in the home are five (5) adult and zero (0) minors. There was a total of three (3) adults present during this inspection, licensee, licensees spouse, and licensee assistant. All adults present at time of inspection are fingerprint cleared and associated to facility in Guardian. Areas accessible to children include: the den/main day care room, 1 bedroom, 1/2 bathroom (near the kitchen), and back yard (fenced) for play. Page 1 of 4 Per licensee, areas off limits to children and parents include 3 bedrooms, 2 bathrooms, living room, dining room, kitchen, garage, and front yard. During this visit, all areas identified on the facility sketch that are accessible for children to use were toured and inspected. Tour began in the den/main day care room where LPA observed age-appropriate toys, equipment, and materials for children. Children use one (1) bedroom during nap time where LPA observed one (1) crib and two (2) play pens that meet title 22 regulations. Children walk through the kitchen to access the bathroom. The children\342\200\231s bathroom is in safe and in sanitary condition. There is a gate that is between the living room, kitchen, and den that is kept closed. LPA observed a washer and dryer inside the bathroom; however, all cleaning products and laundry detergent are inaccessible to children. Currently, children are using the front yard for outdoor play time. Per Licensee, the back yard is being redone and is currently inaccessible to children. The front yard play area was observed to be fenced. LPA observed the outdoor yard to have appropriate toys and other materials for children to use. LPA did not observe any objects that can pose a danger to children in the outdoor yard. Per Licensee, there is supervision at all times when the children are outside. Per licensee, she is part of Child Lane food program. Licensee provides breakfast, lunch, and PM snack for children in care. Per Licensee, there is central air and heating in the home. LPA observed a fireplace in the living room that is screened and has a safety latch making it inaccessible to children in care. Per Licensee, the ill/isolation area is in the first living room near the entrance area. The children will wait on the sofa for parents or authorized representatives to pick them up. There is a sofa available for ill children to rest if needed. Detergents, cleaning compounds, and medications were observed to be inaccessible to children. LPA advised that any poison should be locked under key or combination lock. Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPA did not observe any weapons/firearms, or bodies of water around the premises at time of inspection. Per licensee, there are two (2) dogs and one (1) cat on the premises. The pets are kept in an off-limits bedroom during operating hours. Page 2 of 4 The valve on the required 2A 10BC fire extinguisher indicates fully charged. The fire extinguisher was last serviced on 02/13/2026 and service tag is attached. Smoke and carbon monoxide detectors were tested and were operable on this day. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last fire and disaster drill was conducted on 02/09/2026. The home is observed to be clean and orderly. There are first aid supplies available located in the kitchen. LPA observed that there are age-appropriate toys available for children. Appropriate sleeping arrangements in form cribs, play yards, and mats were observed. Child\342\200\231s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, and LIC 995A Notification of Parents\342\200\231 Rights. Licensee file was reviewed for the following: LIC 508- Criminal Record Statement, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Pediatric First Aid and CPR certification expires 08/2027 for licensee. Mandated Reporter Training has been completed and expires on 03/14/2027 for licensee. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. LPA did not observe the following items during the inspection: Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at Page 3 of 4 https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Clara Recendez, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
