What the inspector wrote

This incident took place at Cortez Child Development Center RM D105 facility number 198013698. Licensing Program Analyst (LPA) Kamile Martin conducted an unannounced Case Management Inspection due to an incident that occurred on 2/25/26 and was reported to the Department on 3/2/26 LPA met with Teacher Bertha Hurtado whom the reason for the visit was explained. LPA went on a self-guided tour of the facility LPA observed preschool classroom D105 indoor area. There are 14 children present and 3 teaching staff present. The facility was observed to be operating within the license capacity limitations. On 3/2/26 an unusual incident report was made to the Department regarding A child (1)shared with her mother when at the lunch table Child (2) touched her inappropriately in her private area. No adults witnessed the incident. Teacher Bertha Hurtado called the mandated reporter hotline and was provided with a claim number. (claim A20260305-441944) And told that this is not reportable incident by Kent Edington. Teacher Bertha Hurtado Disclosed that child (1) sat next to child (2) during lunch time that day. She described the children\342\200\231s temperament and differing skill sets. She states child (2) is more active and has had difficulty with self-control and personal space. Also child (1) has a quiet demeanor and is not as outgoing as peers. Since this incident occurred the children have separated seating arrangements and more supervision placed on ch (2). Teacher disclosed home visits were recently completed at the end of February, and visits went well with both families Mental Health Specialists supported Teacher Bertha on the visit for Ch (2) to provide additional information on the service\342\200\231s mental health offers. During today\342\200\231s visit, LPA spoke with teacher who completed the UIR and observed children in the preschool environment eating snacks, reviewed sign in sheets and toured the classroom environment. LPA observed the classroom environment to be in comp with title 22 regulations. Based on information obtained during the investigation, Policies and procedures were followed . A follow up visit suggested after Spring Break. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00 Exit interview conducted and report was reviewed with Bertha Hurtado.
