Inspector’s narrative

What the inspector wrote

Licensing Program Analysts (LPAs) Alicia Mooberry and Erica Lujan conducted an unannounced Case Management Inspection to address deficiencies observed. LPAs met with Mirna Jarurequi, Licensee and informed of the purpose of inspection. Also present was Rosanna Ramos, Assistant. LPA toured the facility and observed 2 children present. During this visit LPA Mooberry reviewed licensee, staff and child's files. LPA Mooberry observed that the CPR/1st Aid certificate for licensee and staff 1 is not EMSA approved. LPA Mooberry observed that file for Child #1 was not available for review. This poses a potential risk to the health and safety of children in care. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted with Licensee Mirna Jaurequi. Appeal rights were discussed and a copy provided.
