What the inspector wrote

At 1:00PM. The Licensing Program Analyst (LPA), T. Tran, conduct an unannounced site visit at Glazier Head Start to follow up on a Case Management Incident that occurred on 11/17/2025 pertaining to a head injury. Upon arrival, LPA met with Teacher Rosie Carrillo. LPA observed the children were napping. Proper care and supervision observed. During today's visit, LPA completed staff and children files review and obtained the child's document. Interviews were conducted with staff, children, and others. Based on information that was gathered through interviews and record reviewed. On the day of the incident, there were 20 children with 5 teachers. While engaging in an outdoor activity, C1(See LIC 811) was stuck by a pole at the play structure. C1 sustained a small cut in the middle of the forehead which required medical care. Staff immediately provided first aid and contacted the parent. Parent took C1 to Urgent Care and received butterfly Band-Aid to close the wound. C1 was released the same day and there were no restrictions upon returning to school. Based on the available information it does not appear this incident was the result of a Title 22 violation of lack of care and supervision. No further investigation is required for the incident reported. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Rosie Carillo.
