What the inspector wrote

Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection for the purpose of following up on an incident reported to the Department on 02/06/2025. Upon arrival at 8:45am, LPA met with Executive Director Tabetha Turner. At 9am, LPA was led on a tour of the facility. There were children present and census was taken. There were 97 children with 21 staff members. On 02/05/2025, the facility reported that they terminated the membership of a family due to disruptive, inappropriate, and unprofessional behavior towards the facility staff. During an inspection on 02/18/2025, information was provided to LPA regarding what led to the dismissal of the family. Family's child was observed to have a diaper rash that required further medical evaluation. Child injury was not provided in the unusual incident report submitted to Department. Per staff interview on 02/18/2025, the family requested accommodation for their child due to sensitive skin, in the form of specific wipes (provided by family) and that facility not use ointment on child. Upon file review for the child, there was no documented agreement between facility and family noting that they would provide accommodation for the child. Based on the information above, LPA discussed with facility, the importance of documenting any agreement that is being made to assist a child medically. An Advisory Note is being provided to address agreement/accommodation documentation and to address reporting requirements. There are no citations being provided on today's date, in accordance with Title 22 regulations. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Executive Director Tabetha Turner at 10:45am. Copy of Report provided. END OF REPORT
