What the inspector wrote

Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection for the purpose of following up on report submitted to the Department on 04/01/2025. Upon arrival at 8:50am, LPA met with Interim Center Director Ariana Alonzo, to whom the purpose of the inspection was explained. A tour of the facility was provided. There were children present during the time of inspection. Census was taken. There were 82 children with 18 staff members. On 03/26/2025, an incident occurred during a class activity where Child #1 was observed to be showing their private area to Child #2. During today's inspection, Interviews were conducted with three staff members and the two children involved. Video footage of the incident was reviewed and written statement from Staff #2 was obtained. During staff interview, it was explained that during a class activity, the staff member observed Child #1 and #2 moving with their hands under the table and whispering to each other. Staff #2 asked the children what they were doing as it seemed "sneaky" and "inappropriate." Staff #2 stated that Child #2 explained that Child #1 would sometimes pull down their pants and reveal their private area. Per staff interviews, administration was notified and parents were made aware of the situation. Children interviews made no disclosures. LPA reviewed the video footage and written statement. Both evidence corroborated the information from interviews, On the video clip, regarding the incident, LPA observed the children tapping each other and whispering. Child #1 was observed playing with their waist band. Staff #2 was observed catching the incident and responded to the situation effectively and appropriately. Per staff interviews, the children involved are being monitored more closely and but are allowed the freedom to play and interact with each other. REPORT CONTINUES PAGE 1 of 2 Based on the information obtained during inspection, facility responded appropriately to the situation and was positioned with the children where they were able to intercept the behavior and redirect. No violation of Title 22 regulations is being cited during today's inspection. LPA discussed with Interim Director about reporting requirements and the importance of having incidents that directly affect the children in care, submitted immediately within the 24 hr requirement. An Advisory Note is being provided. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Interim Center DIrector Ariana Alonzo, at 11:05am. Copy of report provided. END OF REPORT PAGE 2 of 2
