What the inspector wrote

On September 16, 2025, Licensing Program Analysts (LPAs) Monique Ayala and Mariah Aguirre conducted an unannounced Case Management- Incident inspection at the above facility. LPAs met with director, Ariana Alonzo who guided LPAs on a tour of the facility. LPAs did not observe any children in care at the time of inspection. The purpose of the inspection is to follow up on an incident that occurred on 08/28/2025 and was reported to the department on 08/28/2025; the incident was reported in a timely manner. During the inspection LPAs interviewed Staff #1 (S1) to Staff #3 (S3) , Parent #1 (P1) obtained a facility roster and other relevant documents. LPAs also inspection the interior of the vehicle/van that is exactly the same as the one in the incident. LPAs were unable to interview children as they were not present at the time of inspection. Disclosed in interviews, S3 stated they were driving back to the facility from picking up children from school when a car that had a stop sign did not stop and hit the facilities vehicle/van (rear drivers side). Per S1-S3, there were two children in the vehicle. Per S1-S3, both children had their seat belts on. Per S3, they do not start the vehicle until all children have their seat belts on. LPAs interviewed P1 who stated their child did not require medical attention as the paramedics that were on the site of the accident released the children and staff. Per P1, they continued to monitor symptoms at home. Per S1, parents were notified of the incident. At this time there are no deficiencies being cited as there is a police report pending. LPAs informed director, once the police report is received to forward to the department for review. An exit interview was conducted and a copy of this report was provided to director along with Notice of Site Visit that must be posted for 30 days.
