Incident investigation·October 15, 2025

On October 15, 2025, Licensing Program Analysts (LPAs) Monique Ayala and Mariah Aguirre conducted an unannounced Case Management- Incident inspection at the above facility. LPAs met with director, Ariana Alonzo who guided LPAs on a tour of the facility. LPAs observed 11 school age children with 3 staff members. The purpose of the inspection is to follow up on an incident that occurred on 08/28/2025 and was reported to the department on 08/28/2025; the incident was reported in a timely manner. During the investigation of the incident LPAs interviewed Staff #1 (S1) to Staff #3 (S3) , Parent #1 (P1) obtained a facility roster and other relevant documents. LPAs also inspection the interior of the vehicle/van that is exactly the same as the one in the incident. LPA Ayala obtained a copy of the police report via email on 09/29/2025. During interviews, S3 stated they were driving back to the facility from picking up children from school when a car that had a stop sign did not stop and hit the facilities vehicle/van (rear drivers side). LPA reviewed the police report where it indicated that the other driver was at fault. Per the police report, the driver did stop at the designated stop sign but did not stop to give the right away to the Kids Klub van. There are no deficiencies being cited. An exit interview was conducted and a copy of this report was provided along with Notice of Site Visit. The Notice of Site Visit must be posted for 30 days.
