On November 4, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director, Ariana Alonzo who guided LPA on a tour of the facility. LPA did not observe any school age children at the time of inspection. The purpose of the inspection is to follow up on an incident that occurred on 10/24/2025 and was reported to the department on 10/24/2025; the incident was reported in a timely manner. During the inspection LPA obtained a current facility roster. LPA was unable to interview staff and children as neither were present at the facility. LPA informed director to forward police report once the facility received it. There are no deficiencies being cited at this time. An exit interview was conducted and a copy was provided to director along Notice of Site Visit. Notice of Site Visit must be posted for 30 days.
