What the inspector wrote

*This is an Amended report created by LPA Monique Ayala on 03/03/2026.* On March 3, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director, Ariana Alonzo who guided LPA on a tour of the facility. LPA did not observe any school age children at the time of inspection. The purpose of the inspection is to follow up on an incident that occurred on 10/24/2025 and was reported to the department on 10/24/2025; the incident was reported in a timely manner. LPA obtained a current facility roster, LPA interviewed Staff #1 (S1) to Staff #2 (S2), LPA interviewed Parent #1 (P1). LPA obtained a copy of the police report and a copy of the video footage from the accident. Based on interviews with S1 and S2, they were at at stop sign with children in the van when a car did not stop on time and hit the rear end of the van. Per S1-S2 all children and staff had their seat belts on. LPA observed children and staff wearing their seat belts on the video. LPA observed S1 and S2 look in the rear view mirror when the car hit the rear end of the van. Based on the police report the other vehicle was at fault as they were driving at an unsafe speed while Kids Klub van was at a complete stop causing the other car to hit the Kids Klub van. Per S1-S2 and director there were no injuries sustained. There are no deficiencies cited at this time as the incident was an accident that was caused by the other driver. An exit interview was conducted and a copy of this report was provided to the director along with Notice of Site Visit. The Notice of Site Visit must be posted for 30 days.
