What the inspector wrote

On September 16, 2025, Licensing Program Analysts (LPAs) Monique Ayala and Mariah Aguirre conducted an unannounced Case Management- Incident inspection at the above facility. LPAs met with director, Ariana Alonzo who guided LPAs on a tour of the facility. LPAs observed 18 infants/toddlers in care with 6 staff members. The purpose of the inspection is to follow up on an incident that occurred on 09/10/2025 and was reported to the department on 09/11/2025; the incident was reported in a timely manner. During the inspection LPAs interviewed Staff #1 (S1) to Staff #4 (S4) obtained a facility roster and other relevant documents. LPAs also inspection the area where the incident occurred. LPAs were unable to interview Child #1 (C1) as C1 is an infant and nonverbal. Based on interviews it was disclosed S3 was entering the infant room when C1 was near the door. Per S3, they did not look over that half door to check if any infant was behind the door. Per S3, they opened the door and accidentally hit C1 on the head with the door. Per S4, they were in the classroom with C1 when the incident occurred and observed C1 to be sitting near the door and observed when S3 opened the door. Per S4, C1 was sitting and when S3 opened the door C1 leaned forward as if they were going to start crawling and the incident occurred. Per S1-S4, an ice pack was provided and C1's parents were informed of the incident. Per S1, C1 had a teleconference with a doctor and a doctors note was provided to the facility. LPAs obtained a copy of doctors note. There are no deficiencies being cited at this time as the incident appears to be an accident that occurred quickly and was unable to be prevented. LPAs informed the director as a best practice, have staff look over the half door prior to opening the door to ensure there are no infants behind the door. An exit interview was conducted and a copy of this report was provided to the director along with Notice of Site Visit that must be posted for 30 days.
